The short version

What matters

  • •Convince Keira Metz to seek refuge at Kaer Morhen so she survives, provides Alexander's notes, and aids Lambert.
  • •Refuse Annabelle's plea to fight her, then bring Graham from Oreton to properly break the curse for the highest experience payout.
  • •Dive south into Lake Wyndamer to recover the Enhanced Wolven Armor diagram from the submerged ruins.

Common mistakes

  • •Agreeing to carry Annabelle's bones to Oreton, which allows the Plague Maiden to escape across the mainland and murders Graham.
  • •Letting Keira visit Radovid or fighting her to the death, which results in her execution or Alexander's notes being burned.
  • •Progressing the main story to sail for the Isle of Mists before finishing A Towerful of Mice, causing the quests to fail permanently.

Walkthrough

The area in the order you play it. Tick things off as you go: they tick off in the checklists below too.

  1. 1

    Approach and Shoreline

Take Keira Metz's boat from her cottage or swim across Lake Wyndamer to the Fyke Isle signpost on the shore. As you step onto the overgrown paths past abandoned peasant huts and dead bodies, explore the surrounding waters to gather sunken crates and chests containing runestones, crafting ores, and relics like the Deithwen silver sword. Before heading inland, dive south of the island near the submerged archway marked by a Wolf symbol to loot a chest holding the Enhanced Wolven Armor diagram.

Tip: Crossbow shots instantly kill Drowners in underwater combat.

  2. 2

    Path to the Outer Palisade

Fight your way past packs of Drowners along the banks and confront Rotfiends lurking around the stone palisade wall and ruined gate. Use Keira's Magic Lamp near the swarms of flies to witness three outdoor memory spots detailing the tragic events. Before passing through the gate, search the ruined watchtower structure nearby to loot random crafting and oil diagrams from a crate inside and another by the stairs.

Tip: When a Rotfiend's health drops below 25%, it will pulsate and swell; roll backward immediately or use Aard to avoid the lethal explosion.

  3. 3

    The Lower Tower Levels

Step inside the tower's ground floor amidst bloodstains, swarms of rats, and rotting food, shining the Magic Lamp to observe ghostly visions of guards pillaging, drinking, and murdering the lord's family. Ascend past abandoned living quarters on the second and third floors to view another ghost spot showing Lord Vserad and his followers dying from poison and rat attacks. Before leaving the third-floor bedroom, search the chest resting right next to the stairs leading up to claim 2 diagrams.

Tip: Apply Necrophage Oil and keep your Silver Sword ready for any leftover beasts on the bottom floors.

  4. 4

    Unlocking the Secret Laboratory

Inspect the upper bedroom and locate the wall lever positioned near the bed, then pull it to reveal the secret door leading upstairs to Alexander's hidden Catriona Plague research laboratory. Enter the laboratory filled with cages and dissection tables, and loot the chest against the wall to obtain 2 diagrams, as well as a chest in the rafters of the top attic for 1 oil formula and 2 equipment diagrams. Finally, shine your Magic Lamp to summon the ghost of Annabelle.

Tip: Search the upper rafters carefully before initiating the conversation with Annabelle to ensure you do not miss the top chest.

  5. 5

    Annabelle's Curse

Speak with Annabelle to learn how she was paralyzed by a sleeping potion and eaten alive by rats, then decide whether to trust her plea. If you refuse to take her bones or state you do not trust her, Annabelle reveals herself as a Plague Maiden (Pesta) and attacks; force her outside using Yrden and Specter Oil, then travel to Oreton to fetch Graham so his kiss permanently lifts the curse and kills him. If you agree to take Annabelle's bones to Graham in Oreton, she slays Graham, turns into a Pesta, and escapes to spread disease across the mainland.

Key fight: Annabelle (The Pesta / Plague Maiden)

Tip: Trap Annabelle inside an Yrden circle to make her material, strike with Specter Oil-coated silver attacks, and blast away her fly swarms with Igni.

Watch out: Agreeing to take Annabelle's bones causes her to murder Graham and roam free, permanently losing Annabelle's Bones and awarding fewer quest experience points.

  6. 6

    Confronting Keira Metz

Return to the base of the tower during For the Advancement of Learning as Keira Metz exits carrying Alexander’s laboratory notes. Intercept her and convince her to travel to Kaer Morhen, allowing her to peacefully hand over the notes and survive to help Lambert later. If you allow her to leave for King Radovid, she will be executed in Novigrad, whereas refusing to let her leave with the notes forces a lethal duel where Geralt kills her and burns the research.

Tip: Use Dimeritium Bombs and Yrden to disrupt Keira's spells, and dodge sideways to evade her lightning strikes.

Watch out: Alexander's laboratory notes, A Towerful of Mice, and For the Advancement of Learning will permanently fail if not completed before setting sail to the Isle of Mists.

Choices here

What you decide here and what it leads to.

Annabelle's Plea

Speaking with Annabelle's ghost in the top laboratory during "A Towerful of Mice"

  • Agree to take her bones to Graham→ You receive Annabelle's bones and bring them to Oreton, where Annabelle transforms into her Pesta form, slaughters Graham, and escapes to unleash disease across the land (300 XP)
  • Refuse to carry her bones / Express distrust→ Annabelle reveals her Plague Maiden form and attacks before retreating outside; you fetch Graham from Oreton to kiss her, which breaks the island's curse, lays her ghost to rest, and results in Graham's death (500 XP)

Most players pick: Refuse to carry her bones / Express distrust

Both "A Towerful of Mice" and "For the Advancement of Learning" will permanently fail if you travel to the Isle of Mists prior to finishing them.

Keira Metz and Alexander's Research

Intercepting Keira outside the tower base as she departs with the plague notes during "For the Advancement of Learning"

  • Persuade Keira to seek refuge at Kaer Morhen→ Keira surrenders the research notes, stays alive, and joins the defense during "The Battle of Kaer Morhen," where she prevents Lambert's death (100 XP)
  • Allow her to depart for King Radovid→ Keira journeys to Radovid, who subjects her to torture and impales her corpse on a spike in Novigrad's Hierarch Square, opening the quest "A Final Kindness" (100 XP)
  • Refuse to let her leave with the research / Provoke combat→ You battle and kill Keira in front of the tower, and Geralt incinerates Alexander's papers (200 XP)

Most players pick: Persuade Keira to seek refuge at Kaer Morhen

Siding against Keira initiates a boss encounter right outside the tower.
