← All areas in this guide

The Astral Prism is a secluded demiplane located within the mysterious githyanki artefact that houses Prince Orpheus and the Emperor. Players enter this space across crucial story junctures to discover the true identity of their Dream Visitor, fend off attacks from Prince Orpheus's honour guard, and make pivotal decisions concerning the tadpole's evolution and the fate of the realm. Traversing floating platforms in low gravity, the party must navigate dialogues, resolve loyalties between the mind flayer and the githyanki prince, and protect their mental autonomy against the Absolute.

The short version

What matters

  • •Defeating the Honour Guard to loot the missable Boots of Uninhibited Kushigo from Prelate Lir'i'c.
  • •Choosing whether to consume, commune with, or reject the Astral-Touched Tadpole for partial ceremorphosis.
  • •Deciding whether to assimilate or liberate Prince Orpheus using the Orphic Hammer to determine your endgame allies.

Common mistakes

  • •Attacking the Emperor during the Act 2 to Act 3 ambush, which collapses his protective sphere and instantly causes a Game Over to Netherbrain domination.
  • •Forgetting to loot Prelate Lir'i'c's corpse before stepping through the portal back to Wyrm's Lookout campsite, locking you out of the Boots of Uninhibited Kushigo forever.

What you can skip

  • •Attacking the Dream Visitor during your Act 1 visit, as they will instantly regenerate without altering your storyline.

Walkthrough

The area in the order you play it. Tick things off as you go: they tick off in the checklists below too.

  1. 1

    Face the Dream Visitor

When you enter the planar rift from Crèche Y'llek, follow the floating stone platforms directly into the alien cavern housing the crystalline sphere. Inside, speak with the Dream Visitor, who explains their role in halting your ceremorphosis and hands you a blade to strike them down. You can refuse or attack, but striking them will only prompt them to heal instantly and chide your obedience before you exit back through the rift.

Tip: Take advantage of the native Astral Gravity condition to make massive leaps across the floating terrain without taking any falling damage.

  2. 2

    Repel Orpheus's Honour Guard

After entering the emergency portal during the Act 2 to Act 3 transition, leap down from the entry ridge to aid the friendly Intellect Devourers against the encroaching githyanki. Push forward into the colossal skull dome where the Emperor is holding Prince Orpheus captive, and eliminate the attacking githyanki strike team. Before taking the portal out to the city, ensure you search the battlefield thoroughly.

Key fight: Defend the Dream Visitor (The Emperor)

Tip: Target the Gish'ra monks and prelate quickly with crowd-control spells to prevent them from overwhelming the Emperor and your front line.

Watch out: Prelate Lir'i'c holds the Boots of Uninhibited Kushigo, which are permanently lost if you enter the portal to the city without looting his corpse.

  3. 3

    The Emperor's Offer

Once the Honour Guard is defeated, speak to the Emperor at the center of the skull to discuss his true illithid identity and the captured Prince Orpheus. He presents you with the Astral-Touched Tadpole, offering you heightened powers at the cost of partial ceremorphosis. You can choose to commune with or consume the specimen, but you must pass a DC 21 Wisdom saving throw to refuse it if your character previously consumed any illithid tadpoles.

Tip: If you want the tier-3 illithid powers without permanent cosmetic changes on your main character, place the tadpole in your inventory to offer it to companions instead of consuming it directly.

Watch out: Crushing or refusing the tadpole after having consumed mind flayer parasites requires beating a steep DC 21 Wisdom check, which forces partial transformation on a failure.

  4. 4

    The Final Incursion

When routed to the Astral Prism following the showdown at the Morphic Pool, gather your party inside the crystalline dome to make a final stand with the Netherstones. You must decide whether to give the stones to the Emperor so he can assimilate Prince Orpheus, or reject the mind flayer to free the githyanki prince yourself. If you side with the Emperor, prepare for Lae'zel to turn permanently hostile unless you talk her down with a DC 30 Persuasion check.

Tip: If you plan to side with the Emperor and keep Lae'zel in your party, pre-cast Guidance and stock Inspiration points to handle the exceptionally high DC 30 Persuasion check.

Watch out: Attacking the Emperor unprovoked or defying Raphael without having acquired the Orphic Hammer will leave you with no viable way to dominate the Netherbrain, triggering an immediate Game Over.

  5. 5

    Shatter Orpheus's Bonds

If you reject the Emperor and choose to free Prince Orpheus, watch the Emperor depart to join the Netherbrain before taking up your weapon. Equip the Orphic Hammer and strike both infernal binding crystals flanking Orpheus's psychic prison to shatter the field. Once free, converse with Orpheus to decide who will make the ultimate sacrifice to undergo ceremorphosis and wield the Netherstones before stepping through the exit portal to High Hall.

Tip: Equipping the Orphic Hammer on a high-strength warrior breaks each binding crystal in a single strike.

Watch out: If you side against the Emperor without possessing the Orphic Hammer, you must sign Raphael's desperate pact right inside the dome, or your journey will instantly end in failure.

Choices here

What you decide here and what it leads to.

Kill or Spare the Dream Guardian

Act 1, Crèche Y'llek inside the Prism

  • Strike the Guardian→ you discover they instantly regenerate, mildly damaging their trust
  • Refuse to strike→ the Guardian commends your resolve and reveals Vlaakith fears the artifact's true power

Most players pick: Refuse to strike

If Lae'zel is present, defying Vlaakith's command requires passing dialogue checks to prevent Lae'zel from becoming hostile

Fate of the Emperor vs. Honour Guard

Act 2-to-Act 3 long rest ambush

  • Defend the Emperor against the Githyanki Honour Guard→ you survive the assault and learn the Emperor's true identity
  • Attack or side against the Emperor→ the Emperor drops his psionic protection, causing an immediate Game Over as your party transforms into thralls of the Netherbrain

Most players pick: Defend the Emperor against the Honour Guard

Entering the portal at camp has a 3-turn countdown timer before a Game Over occurs

The Astral-Touched Tadpole

Immediately following the Act 2/Act 3 ambush

  • Commune with or consume the tadpole→ gain partial-illithid status, unlocking top-tier illithid powers and cosmetic black facial veins
  • Decline or stow the tadpole→ keep your mortal appearance and avoid ceremorphosis

Most players pick: Decline or stow the tadpole

If your character has already consumed regular tadpoles throughout the game, refusing requires succeeding a DC 21 Wisdom saving throw

Who Controls the Netherstones

Act 3 climax, following the Morphic Pool retreat

  • Side with the Emperor→ the Emperor assimilates Orpheus's brain, remains your primary endgame ally, and Voss becomes hostile unless persuaded
  • Refuse the Emperor and free Orpheus→ the Emperor abandons you to join the Netherbrain (becoming a boss in the final battle), and you use the Orphic Hammer to shatter Orpheus's crystal bindings

Most players pick: Free Orpheus (if carrying the Orphic Hammer)

A point of no return for the endgame; keeping non-Vlaakith Lae'zel loyal if you let the Emperor absorb Orpheus requires passing a DC 30 Persuasion check, or she turns permanently hostile

Who Becomes a Mind Flayer for the Finale

Act 3 climax, after freeing Orpheus or keeping the stones

  • Prince Orpheus sacrifices himself→ Orpheus transforms into an illithid to wield the stones, sparing your party ceremorphosis
  • The player character or Karlach volunteers→ the chosen character permanently transforms into a mind flayer (preventing Karlach from dying from her engine)
  • Gale volunteers his Netherese orb→ Gale pledges to detonate the orb atop the Netherbrain, leaving mortal forms intact

Most players pick: Prince Orpheus sacrifices himself (or Karlach transforms to save her life)

Permanent, irreversible character transformation that dictates epilogue options and companion romance endings

Tactics: Rush forward through the giant skull entrance to reach the central platform; prioritize crowd-controlling or bursting down spellcasters like Prelate Lir'i'c before they establish Sanctuary or lock down party members, use AoE spells across the central chokepoints, and protect the Emperor to avoid game over

Rewards: Astral-Touched Tadpole (offered by the Emperor) and progression into Act 3

Enemies

Prelate Lir'i'c
Steal / drop Boots of Uninhibited Kushigo

Tips

  • While inside the Astral Prism, your party benefits from Astral Gravity, which triples jump distance and prevents any falling damage, allowing you to easily leap across wide gaps between floating islands.
