← All areas in this guide

The Undercity Ruins is a cavernous subterranean zone reached through the Lower City Sewers that leads directly to the Temple of Bhaal and the Ancient Lair. Adventurers must breach a sealed blood door and navigate the deadly Manslaughter Chasm ambush before accessing the temple gates. Progressing here is necessary to pursue Orin the Red, claim her Netherstone, and save her hostage.

The short version

What matters

  • •Voiceless Penitent Bareki sells premier endgame gear for Druids and Barbarians (Armour of Moonbasking and Bonespike Helmet).
  • •Slaying Strangler Luke before Farslayer Ghislev dies lets you claim the extremely missable Bonespike Gloves.
  • •Looting Wilting Alex's corpse past the chasm secures Dribbles' Severed Head for Lucretious's quest reward, the Spellmight Gloves.
  • •Shooting the hanging corpses above the ritual circle bypasses the sealed door without needing the Murder Tribunal finished first.

Common mistakes

  • •Killing Farslayer Ghislev too quickly; doing so despawns Strangler Luke immediately, locking you out of the Bonespike Gloves permanently.
  • •Getting spotted stealing the pouch behind Bareki; being seen pilfering Sarin's Skull turns him permanently hostile.
  • •Failing to interrupt Ghislev before his Power Word: Kill ritual finishes; use multi-hit attacks to strip his Unstoppable charges quickly so high-damage hits can execute him.

What you can skip

  • •Chasing down every high-perched archer in the chasm is unnecessary; slaying Ghislev ends the encounter and despawns the remaining ambushers automatically.

Walkthrough

The area in the order you play it. Tick things off as you go: they tick off in the checklists below too.

  1. 1

Undercity Ruins Waypoint & Bareki's Alcove

When you pass through the northern sewer archway into the Undercity Ruins, you immediately activate the Undercity Ruins waypoint at `X: -140, Y: 940`. Head directly west to `X: -180, Y: 935` to find the mute vendor Voiceless Penitent Bareki standing outside the passage to the Ancient Lair. Trade with him to browse unique gear like the Armour of Moonbasking and Bonespike Helmet before attempting anything suspicious around his camp.

Tip: Tip: Bareki carries elite Druid and Barbarian gear; buy what you need before touching his belongings.

Watch out: Warning: Attacking Bareki or getting caught stealing will cause him to turn hostile or die permanently, locking you out of his shop.

  2. 2

    Claiming Sarin's Skull

Sneak behind Voiceless Penitent Bareki at `X: -156, Y: 946` where a small pouch rests on the stone steps. Enter turn-based mode while invisible or distract him in conversation with one party member so you can steal the pouch unnoticed. Inside you will find Sarin's Skull, which causes a silent, headless ghost to follow whoever carries it until it is laid to rest elsewhere.

Tip: Tip: Separate one stealthy companion under Greater Invisibility or a Potion of Invisibility to pocket the skull without involving the entire group.

Watch out: Warning: Interacting with the pouch counts as theft; if Bareki spots you, he turns permanently hostile.

  3. 3

    Unlocking the Ritual Stone Door

Return east toward `X: -135, Y: 934` where a massive stone door marked with Bhaalist ritual sigils seals the path leading north. You can bypass the lock cleanly by presenting the Amulet of Bhaal, or using ranged attacks to shoot either of the two hanging corpses suspended from ropes above the ritual circle so their blood spatters onto the floor plate below. Once the blood pools, the stone threshold grinds open, revealing the cavern beyond.

Tip: Tip: A simple bow shot or cantrip aimed at the hanging bodies bypasses this barrier immediately without burning a spell slot or requiring the Amulet of Bhaal.

  4. 4

    Surviving the Manslaughter Chasm Ambush

Cross the threshold into Manslaughter Chasm at `X: -100`, where passing the dagger-pinned warning note triggers an ambush by Bhaalist snipers and Farslayer of Bhaal Ghislev. Ghislev immediately begins chanting Bhaal's Power Word: Kill on a strict five-turn countdown while protected by stacks of Unstoppable. Send mobile skirmishers across the broken pillars or use multi-hit attacks like Magic Missile and Upcast Scorching Ray to strip Ghislev's charges and burn his health down before the timer expires, as his Ethereal and Unstoppable traits prevent him from being shoved or displaced.

Key fight: Ambush at Manslaughter Chasm

Tip: Tip: Multi-target and multi-hit spells peel through Ghislev's Unstoppable charges instantly so high-damage melee hits can execute him.

Watch out: Warning: If Ghislev completes his 5-turn ritual, your entire party is instantly wiped out by Power Word: Kill.

  5. 5

Eliminating Strangler Luke for the Bonespike Gloves

During the Manslaughter Chasm skirmish, locate the melee cultist Strangler Luke advancing near the bridge before you finish off Ghislev. Direct your party to focus down Luke, then move in to loot the Bonespike Gloves directly from his body. Once the gloves are safely gathered, proceed to eliminate Ghislev to officially break the ambush trial.

Tip: Tip: Use misty step or telekinesis to quickly bridge the gap to Luke's roost so you can burst him down before Ghislev falls.

Watch out: Warning: Slaying or shoving Ghislev immediately ends the trial and despawns every surviving cultist, permanently forfeiting the Bonespike Gloves if Luke is alive.

  6. 6

The Blast Mine Descent & Wilting Alex

Push north past the ambush grounds toward `X: -65, Y: 1041` through the solid metal door leading to the path outside the temple gates. Watch the ground carefully for clusters of chained corpses rigged with volatile blast mines. Disarm or detonate the blast mines from a safe distance, then search the corpse of Wilting Alex to recover the Clown's Severed Head.

Tip: Tip: Detonate the blast mines using a fire arrow or cantrip from afar to safely clear the corpse pile without harming your team.

Watch out: Warning: Rushing into the pile of corpses blindly can trigger a chain reaction of blast mines that downs low-health companions.

Key fights

Ambush at Manslaughter Chasm

Enemies: Farslayer of Bhaal Ghislev, Strangler Luke, and an ambush squad of Bhaalist Reapers, Invokers, and Snipers

Threats: Ghislev channels Bhaal's Power: Kill (an unavoidable wipe of anyone caught in the chasm when his 5-turn ritual countdown completes) and Bhaal's Edict; Snipers use Roaring Thunder arrows from high-ground perches to knock party members into the bottomless chasm; Strangler Luke teleports and restricts movement

Weaknesses and resistances: Ghislev has multiple stacks of Unstoppable (reducing hits to minimal flat damage until removed) and Sanctuary, but possesses low base hit points once Unstoppable is stripped; cultists are vulnerable to being displaced over chasms with Force damage/Telekinesis

Tactics: Use Invisibility, Dimension Door, Fly, or Arrows of Transposition to bypass the open bridge chokepoint and close the distance to Ghislev's high cliff platform; rapidly break his Unstoppable stacks using multi-hit abilities (Upcast Magic Missile, Scorching Ray, or extra attacks) or push/throw him off the cliff to instantly end the encounter and cause the remaining ambushers to flee (ensure Strangle

Rewards: Bonespike Gloves (looted from Strangler Luke before Ghislev is killed or flees), generic Bhaalist gear, access to the Temple of Bhaal.

Enemies

Farslayer of Bhaal Ghislev

Begins chanting a ritual version of Power Word Kill that wipes out the party within five rounds if he is not eliminated.

Strangler Luke
Steal / drop Bonespike Gloves

Despawns alongside other ambushers immediately once Ghislev is killed, requiring players to target and loot him first.

Tips

  • Bypass the hazardous Manslaughter Chasm gauntlet entirely by casting Invisibility or drinking an Invisibility Potion on a mobile runner to reach and activate the Temple of Bhaal waypoint, allowing the rest of the party to fast-travel past the encounter.
