Boarding the boat at the Isle of Mists cutoff: Permanently fails incomplete regional side quests, locking out the "Full Crew" achievement if recruits are not gathered, and voids any unfinished prerequisites for "Assassin of Kings" and missable Gwent cards tied to those NPCs.
Completing "Dead Man's Party" without finishing every wedding event: Permanently locks out the "Shopaholic" and "Let the Good Times Roll" achievements from Hearts of Stone.
Defeating the Iris nightmare clones sequentially instead of waking all six at once in the Von Everec Estate: Permanently locks out the "When It's Many Against One…" achievement.
Killing Golyat without a direct crossbow eye shot at the mill in Toussaint: Permanently misses the "David and Golyat" achievement for that playthrough.
Failing any of the tournament competitions during "The Warble of a Smitten Knight": Permanently misses the "A Knight to Remember" achievement.
Advancing past "The Night of Long Fangs" without keeping a manual backup save: Permanently prevents acquiring both mutually exclusive ending achievements ("Last Action Hero" and "Kling of the Clink") in a single run.

Recommended order

Step 1

Start a fresh campaign on Death March difficulty to clear all difficulty achievements simultaneously without lowering it at any point.
Step 2

Explore White Orchard thoroughly, purchasing initial Gwent cards from the tavern keeper and Elsa, and play the scholar at the tavern to secure the first missable card.
Step 3

Progress through Velen and Novigrad, completing Keira Metz's questline peacefully to send her to Kaer Morhen, attending the Vegelbud masquerade ball to win the three exclusive tournament Gwent cards, and pursuing Radovid's conspiracy side quests.
Step 4

Complete the Skellige succession questline by choosing to back either Cerys or Hjalmar, and recruit all required allies (Keira, Triss, Roche, Ves, Zoltan, Hjalmar, Ermion) across regions for Kaer Morhen.
Step 5

Secure all remaining base-game Gwent cards and win the high-stakes tournament at the Passiflora before triggering the Isle of Mists story transition.
Step 6

Complete the main story through the Battle of Kaer Morhen and finish Radovid's assassination questline ("Reason of State") to wrap up base-game story milestones.
Step 7

Begin the Hearts of Stone expansion, making a hard save before attending the wedding during "Dead Man's Party" to complete all festive activities, purchasing all auction house items, and provoking all six Olgierd nightmare clones simultaneously at the Von Everec Estate.
Step 8

Start the Blood and Wine expansion, shoot Golyat directly in the eye with a crossbow during his opening boss fight, win every competition at the Tourney Grounds flawlessly, display the five chivalric virtues, and craft Grandmaster Witcher gear.
Step 9

Create a manual save during "The Night of Long Fangs" before choosing between the Unseen Elder's path and Syanna's fairy tale path; complete one path to obtain its specific ending achievement, then reload to finish the alternative branch.

Kingmaker Missable 24.6% of players Hidden achievement Complete the Skellige succession subplot by finishing "Possession," "The Lord of Undvik," and "King's Gambit" to crown either Cerys or Hjalmar. Where: Kaer Trolde Castle

Full Crew Missable 18.9% of players Hidden achievement Recruit all seven possible allies—Keira Metz, Triss, Roche, Ves, Zoltan, Ermion, and Hjalmar—to defend Kaer Morhen during the "Brothers in Arms" questline. The achievement unlocks once everyone gathers at Kaer Morhen prior to the battle against the Wild Hunt. Where: Kaer Morhen

Assassin of Kings Missable 15% of players Hidden achievement Participate in the assassination plot against King Radovid by completing the secondary quest "Reason of State" in Novigrad. This requires finishing the prerequisite conspiracy questlines with Dijkstra and Roche earlier in the story. Where: Novigrad

Last Action Hero Missable 13% of players Be decorated with the Order of Vitis Vinifera. Complete the Blood and Wine expansion with an ending where Geralt is awarded the Order of Vitis Vinifera by Anna Henrietta. To achieve this, retrieve Syanna from the Land of a Thousand Fables and prevent her death so the Duchess remains alive. Where: Beauclair Palace

Woodland Spirit Missable 12.5% of players Complete the contract on the Woodland Spirit. Pick up the contract "In the Heart of the Woods" in Fayrlund on Ard Skellig and choose Sven's path to hunt the ancient Leshen. Track down and slay the Woodland Spirit rather than performing the ritual exile for the village elders. Where: Skellige Isles

Embodiment of the Five Virtues Missable 12% of players Be given Aerondight by the Lady of the Lake. Demonstrate the five chivalric virtues (Compassion, Generosity, Honor, Valor, and Wisdom) through your dialogue choices and side quests across Toussaint. Meet the Hermit at Lac Céleste in the Sansretour Valley, defeat him in a duel, and receive the silver sword Aerondight from the Lady of the Lake. Where: Sansretour Valley

A Knight to Remember Missable Collectible 11.3% of players Obtain a flawless victory in all the competitions during the knights' tourney. Enter the knights' tourney during the side quest "The Warble of a Smitten Knight" at the Tourney Grounds. Achieve first place in every event—the shooting range, horseback race, arena skirmish, and duel against the reigning champion—without failing or withdrawing. Where: Tourney Grounds

Gwent Master Missable 11.1% of players Defeat Tybalt and win the gwent tournament held at the Passiflora. Enter the high-stakes Gwent tournament at the Passiflora brothel in Novigrad during the quest "High Stakes." Win all four consecutive matches without losing a single game, defeating Count Tybalt in the final round.

Curator of Nightmares Missable 10.9% of players Hidden achievement Recreate all eight of Iris von Everec's restored memories while exploring the Painted World during the main quest "Scenes From a Marriage". Be sure not to overlook the optional, missable memory located at the gazebo directly by the entrance garden. Where: Painted World

Fast and Furious Missable 9.7% of players Win all the horse races in the game. Win every horse race tournament across Velen, Novigrad, and the Skellige Isles, including the final race against Cleaver's rider during "Race: The Great Erasmus Vegelbud Memorial Derby." Where: Novigrad

Shopaholic Missable 9.5% of players Hidden achievement Attend the auction during the Hearts of Stone heist quest "Open Sesame!" inside the Borsodi Auction House. Place winning bids on all three items put on the auction block: the brass figurine, the painting, and the spectacles. Where: Borsodi Auction House

The Grapes of Wrath Stomped Missable 9.5% of players Hidden achievement Start the side quests "Wine Wars: Coronata" and "Wine Wars: Vermentino," discover the conspiracy notes in both vineyards before turning either quest in, and complete "Wine Wars: The Deus in the Machina". Uniting Liam and Matilda allows them to jointly run Belgaard Vineyard, after which they will name a wine in your honor. Where: Coronata Vineyard

Even Odds Missable 9.4% of players Kill 2 monsters you have a contract on without using Signs, potions, mutagens, oils or bombs. Complete any two Witcher Contracts and slay their target monsters without using Signs, potions, mutagens, oils, or bombs.

Let the Good Times Roll! Missable 8.5% of players Hidden achievement During the Hearts of Stone quest "Dead Man's Party", participate in every single mandatory and optional wedding activity before midnight strikes.

Card Collector Missable Collectible 4.8% of players Acquire all gwent cards available in the base version of the game. Obtain every base-game Gwent card across White Orchard, Velen, Novigrad, and Skellige before questlines lock out certain players and merchants.

David and Golyat Missable 3.2% of players Hidden achievement When battling the giant Golyat at the Dulcinea Windmill upon arriving in Toussaint, kill him with a crossbow bolt aimed directly into his exposed eye.

Kling of the Clink Missable 3% of players Serve time in Toussaint. Achieve the prison ending in Blood and Wine by letting Syanna die during the story and completing the ensuing jail quest. Where: Toussaint Prison

When It's Many Against One… Missable 2.6% of players Hidden achievement In the quest "Scenes from a Marriage", strike and provoke all six of Iris von Everec's ethereal Nightmares so they fight you simultaneously, then defeat them. Where: Painted World

Lilac and Gooseberries 60% of players Hidden achievement Complete the prologue quest "Lilac and Gooseberries" by finding Yennefer in White Orchard. Where: White Orchard

Butcher of Blaviken Cumulative 50.8% of players Kill at least 5 opponents in under 10 seconds. Slay at least 5 enemies within 10 seconds, which is easily accomplished against swarms of low-level wolves, dogs, or drowners using bombs or the Igni Sign.

Fist of the South Star 49.3% of players Defeat an opponent in a fistfight without taking any damage. Win any fistfight without receiving a single blow by parrying and countering your opponent's attacks.

Let's Cook! 44.6% of players Learn 12 potion formulae. Find or purchase and read 12 unique potion recipes to learn them.

Can't Touch This! Cumulative 42.5% of players Kill 5 foes in a fight without taking damage (except for Toxicity) and without using the Quen Sign. Defeat 5 foes in a single encounter without taking damage or casting Quen, easily done against low-level packs using ranged attacks or bombs.

Bookworm 39.8% of players Read 30 books, journals or other documents. Pick up and read 30 distinct books, notes, journals, or letters from your inventory.

Family Counselor 37.1% of players Hidden achievement Progress through the main story quests in Velen to locate the Bloody Baron's wife, Anna, and his daughter, Tamara. Where: Crow's Perch

Bombardier 32.6% of players Collect the formulae for 6 different bomb types. Acquire and learn the crafting formulae for 6 distinct bomb varieties from herbalists or loot chests.

Triple Threat 31.1% of players Hidden achievement Slay three separate opponents in a single combat encounter using three distinct attack methods, such as a sword strike, a Sign, and a crossbow bolt.

A Friend in Need 30.3% of players Hidden achievement Rescue Dandelion from the witch hunters during the main quest "A Poet Under Pressure." Where: Novigrad

Shrieker 30.2% of players Complete the contract on the shrieker. Pick up "Contract: Shrieker" from the Crow's Perch notice board and defeat the cockatrice nesting in the nearby caves. Where: Crow's Perch

Necromancer 28.9% of players Hidden achievement Aid Yennefer in performing necromancy on Skjall's remains during the main story quest "Nameless" in Hindarsfjall. Where: Garden of Freya

Environmentally Unfriendly Cumulative 28.7% of players Kill 50 opponents using the environment (e.g. swamp gas, insects or objects). Kill 50 enemies using hazard sources in the world, such as detonating swamp gas pockets with Igni or igniting explosive barrels.

Mutant 26.1% of players Fill all mutagen slots. Unlock all four mutagen slots in the character development tree and equip a mutagen into each.

Fire in the Hole Cumulative 24.9% of players Destroy 10 monster nests using bombs. Destroy a total of 10 monster nests across the world by clearing them and dropping a bomb inside.

Something More 24.4% of players Hidden achievement Progress through the main story until you travel to the Isle of Mists and successfully locate Ciri.

Globetrotter 24.3% of players Discover 100 fast travel points. Discover and unlock 100 fast travel signposts across the game's various maps.

Munchkin 24% of players Reach character development level 35. Gain experience points through quests and combat until Geralt reaches character level 35.

The Doppler Effect 23.5% of players Resolve the doppler problem in Novigrad. Complete "Contract: An Elusive Thief" in Novigrad by hunting down the doppler and either sparing or slaying him. Where: Novigrad

Xenonaut 23.3% of players Hidden achievement Accompany Avallac'h through worlds during the main quest "Through Time and Space" to reach Tir ná Lia and speak with Ge'els.

Fearless Vampire Slayer 22.3% of players Complete the contract on Sarasti. Complete "Contract: The Mystery of the Byways Murders" and slay the ekimmara Sarasti inside the Byways catacombs. Where: Byways

The King is Dead 21.9% of players Hidden achievement Defeat Eredin Bréacc Glas during the climactic main story quest "On Thin Ice." Where: Undvik

Passed the Trial Difficulty 21.9% of players Finish the game on any difficulty. Complete the main story campaign of the base game on any difficulty setting.

Ashes to Ashes 19.7% of players Complete the contract on Therazane. Pick up the contract "Haunted House" from the notice board in Hierarch Square to start "Contract: Doors Slamming Shut". Slay the earth elemental Therazane in the Moldavie Residence cellar and collect your reward. Where: Hierarch Square

The Witcher's Gone South 19% of players Travel to the Duchy of Toussaint. Travel to the Duchy of Toussaint by completing the Blood and Wine prologue quest "Envoys, Wineboys" in Velen. You arrive in Toussaint alongside the ducal knights to trigger the achievement. Where: Velen

I'm Not Kissing That 18.7% of players Hidden achievement Progress through the main Hearts of Stone quest "Evil's Soft First Touches" and enter the sewers beneath Oxenfurt. Slay the cursed Toad Prince boss residing in the depths. Where: Oxenfurt Sewers

Fiend or Foe? 18% of players Complete the contract on Morvudd. Take the "Contract: Missing Son" notice in Fyresdal on Ard Skellig to track down the fiend Morvudd. Defeat Morvudd at its lair in the ruins and claim your bounty. Where: Skellige Isles

Armed and Dangerous Collectible 17.1% of players Find and equip all the elements of one set of witcher gear. Craft and equip all elements of a single witcher gear set simultaneously, including all four pieces of armor as well as both the steel and silver swords.

Geralt and Friends 16.3% of players Win a round of gwent using only neutral cards. Win any round of Gwent using only neutral cards such as Geralt, Ciri, Dandelion, neutral spies, decoys, horns, or weather cards without playing any faction-specific cards.

Pacta Sunt Servanda 16.3% of players Hidden achievement Complete the Hearts of Stone expansion storyline by finishing the quest "Whatsoever a Man Soweth..." at the Temple of Lilvani. The achievement unlocks whether you choose to challenge Gaunter O'Dimm or let him claim Olgierd's soul. Where: Temple of Lilvani

Brawler 15.4% of players Defeat Olaf, the Skellige champion of unarmed combat. Complete the preceding Skellige fistfighting matches in the quest "Fists of Fury: Skellige" to earn the right to fight Olaf on An Skellig. Defeat Olaf the bear in unarmed combat to claim victory. Where: Skellige Isles

All In 15.3% of players Play three hero cards in one round of gwent and win the match. Place three Hero cards onto the board within a single round of Gwent and proceed to win the overall match.

Weapon "W" 14.8% of players Develop a mutation. Complete the Blood and Wine secondary quest "Turn and Face the Strange" to unlock Professor Moreau's laboratory. Use ability points and mutagens at the mutation apparatus to develop any single new mutation. Where: Beauclair

Brawl Master 13.7% of players Complete all fistfighting quests in Velen, Skellige and Novigrad. Complete every fistfighting quest across Velen, Novigrad, and Skellige, followed by defeating the champion of champions in the arena on Spikeroog. Where: Skellige Isles

Rad Steez, Bro! 13.1% of players Hidden achievement Find a long, steep mountainside slope (such as on the mountains of Ard Skellig) and slide continuously downhill for at least 10 unbroken seconds. Where: Skellige Isles

Playing House 13.1% of players Use all available options for developing Corvo Bianco. Speak with your majordomo Barnabas-Basil at Corvo Bianco and purchase every available estate upgrade, including the house renovation, grounds improvements, and vineyard additions. Where: Corvo Bianco

Geralt: The Professional 10.1% of players Hidden achievement Complete all 26 base-game monster contracts found on notice boards across White Orchard, Velen, Novigrad, and Skellige.

Dressed to Kill Collectible 9.5% of players Unlock the bonus for equipping all the witcher gear elements from one School. Craft and equip a full Grandmaster or Witcher gear set consisting of all armor pieces and both swords from the same Witcher School to trigger the set bonus.

Pest Control 8.4% of players Destroy all monster nests in the Velen / Novigrad region, or in Skellige. Destroy all monster nests located across the Skellige Isles, or alternatively clear every nest throughout the Velen and Novigrad regions.

Dendrologist Collectible 7.6% of players Acquire all the Abilities in one tree. Invest ability points to unlock all 10 abilities across any single skill tree branch. You can use a Potion of Clearance to reallocate points quickly.

Turned Every Stone Collectible 7.6% of players Find all grandmaster diagrams for each witcher school. Complete all Grandmaster Witcher Scavenger Hunts in Toussaint and collect every diagram for the Wolf, Griffin, Cat, Bear, and Manticore schools.

I Have a Gwent Problem Collectible 7% of players Collect all the cards in the Skellige deck. Collect all 19 cards belonging to the Skellige faction deck introduced in the Blood and Wine expansion by completing the tournament and beating local players.

Wild Rose Dethorned 6.7% of players Hidden achievement Locate and defeat all Fallen Knights at the 8 bandit camp hideouts in northeastern Novigrad/Velen, looting the leader's chest at each site.

Ran the Gauntlet Difficulty 6.5% of players Finish the game on the "Blood and Broken Bones!" or "Death March!" difficulty levels. Complete the main story campaign on either the "Blood and Broken Bones!" or "Death March!" difficulty settings without ever lowering the difficulty.

Killed It 5.4% of players Win a round of gwent with a total strength of at least 187. Win any single round of Gwent with a combined board strength score of 187 points or higher.

Power Overwhelming 5.1% of players Have all possible Place of Power bonuses active at the same time. Draw power from all five distinct Place of Power shrine types (Aard, Igni, Yrden, Quen, and Axii) so all their buffs remain active simultaneously. Where: White Orchard

Kaer Morhen Trained 4.1% of players Perform 10 effective counterattacks in a row without getting hit or parrying. Successfully execute 10 parry counterattacks in a row against an attacking foe without getting hit or blocking normally.

Can Quit Anytime I Want 3.8% of players Be under the influence of seven potions or decoctions at the same time. Increase your maximum Toxicity through character skills and drink enough potions and decoctions so that seven distinct effects are active concurrently.

Walked the Path Difficulty 3.6% of players Finish the game on the "Death March!" difficulty level. Complete the entire main story campaign on the hardest difficulty, "Death March!", without lowering it at any point.

Return to Sender Cumulative 3.4% of players Kill 3 opponents with their own arrows. Unlock the Arrow Deflection skill (level 2) in the Combat tree and time your parries to reflect incoming hostile arrows back to kill three archers.

That Is the Evilest Thing… Cumulative 3.1% of players Ignite the gas produced by a Dragon's Dream bomb using a burning opponent. Do this 10 times. Ignite the flammable cloud of a Dragon's Dream bomb using an enemy that is already actively burning, repeating this 10 times.

Overkill Cumulative 3% of players Make an opponent suffer from bleeding, poisoning and burning simultaneously. Do this 10 times. Inflict bleeding, poisoning, and burning status effects on a single living opponent at the exact same time, repeating this for 10 enemies.

What Was That? 3% of players Hidden achievement Perform a basic sword attack, a parry counter, cast any Sign, and throw any bomb in any sequence within 4 seconds.

Moo-rderer 2.9% of players Hidden achievement Slay 20 domesticated cows found roaming pastures and farms across the game world.

I Wore Ofieri Before It Was Cool Collectible 2.7% of players Collect all available Ofieri armor and horse gear, and at least one Ofieri sword. Acquire every piece of Ofieri armor, horse gear (saddle, bliders, saddlebags), and at least one Ofieri sword from the Hearts of Stone expansion.

The Enemy of My Enemy Cumulative 2.6% of players Use the Axii Sign to force one opponent to kill another. Do this 20 times. Equip the Puppet alternate mode for the Axii Sign and brainwash enemies so that they kill other hostiles 20 times.

Hasta la Vista™ 2.2% of players Hidden achievement Freeze an opponent using a Northern Wind bomb, then eliminate them with a crossbow bolt while they remain frozen.

Humpty Dumpty Cumulative 2% of players Kill 10 opponents by knocking them off somewhere high with the Aard Sign. Cast the Aard Sign to blast ten opponents off high ledges or cliffs so the resulting fall kills them.

Master Marksman Cumulative 1.8% of players Kill 50 human and nonhuman opponents by striking them in the head with a crossbow bolt. Deliver the fatal blow to 50 human or nonhuman enemies by hitting them directly in the head with crossbow bolts.
