Joel wakes up in Saint Mary's Hospital after being rescued and learns that the Fireflies plan to perform a lethal surgery on Ellie to develop a vaccine. Armed only with the gear he gathers on the way, Joel must fight or sneak past heavily armed Firefly soldiers across multiple floors to reach the pediatric surgery room. After taking Ellie from the operating table, Joel escapes through the building while evading pursuing guards to reach the parking garage.

Unlocking the locked storage room door using a Shiv on the sixth floor rewards a significant cache of crafting materials, weapon parts, and ammunition.

Firefly Assault Guard

Weak to Headshots bypass armor helmets if struck with high-powered roundsSteal / drop Assault Rifle ammunition

Equipped with body armor and automatic rifles; they coordinate aggressively upon detecting Joel.

Tips

Smoke bombs are exceptionally effective in the long hospital hallways for bypassing large groups of armored Fireflies without engaging in prolonged firefights.
You only need to incapacitate the lead surgeon in the operating room to pick up Ellie; the other two surgical assistants are completely optional to engage.
During the final sprint carrying Ellie, sprint continuously along the designated corridor path without stopping to avoid being caught and gunned down by pursuing Fireflies.
