During the Blood Moon phase, accessing the rear of the clinic via the Forbidden Woods cave path reveals the impostor Iosefka in a vulnerable state. She is found upstairs on an operating table writhing in pain and will not retaliate when attacked. Defeating her yields a crucial third of an umbilical cord needed for the secret ending.

Tips

Do not kill Impostor Iosefka before the Blood Moon rises (defeating Rom), or she will fight back aggressively and will not drop the One Third of Umbilical Cord.
Any NPCs you sent to the clinic earlier in the playthrough will now permanently be transformed into small Celestial Minions holding the items those NPCs would normally drop.
