The ambush in "Search and Destroy": You must ignore Johnny's prompt to flee through the corridor and instead double back through the hole in the wall to rescue Goro Takemura, or "The Devil" ending achievement becomes permanently locked out for the rest of the playthrough.
Entering the elevator at Embers during "Nocturne Op55N1": Reaching the rooftop meeting with Misty locks you into the endgame assault sequences, so you must create a manual save point here to reload and experience alternate ending pathways.
Dialogue choices in "Chippin' In" and Rogue/Kerry side jobs: Being hostile to Johnny at his unmarked grave or rejecting Rogue and Kerry's requests cuts off the remaining rockerboy questline, permanently locking out "Bushido and Chill" and Johnny's replica clothing items for "Breathtaking".
Deciding to betray Songbird or Reed during "Firestarter" (Phantom Liberty): Backing either Songbird or Reed branches the expansion toward incompatible final missions and achievements, requiring a hard save before making the matrix choice to complete all expansion endings in one run.

Konpeki Plaza - Execute the heist during "The Heist," ensuring you grab Yorinobu's clothes and the iconic weapon on the penthouse roof if desired before escaping into the wider open world.
