Escaping the dungeons in Prologue: "Eagle Eye" and completing the ballista/stealth challenges in La Valette Castle.
Boarding the barge at the end of Chapter 1: All Flotsam side quests, fistfighting, poker, arm wrestling, and the choice between Roche's and Iorveth's paths.
Dispelling the blood mist in Chapter 2: All Vergen or Kaedweni Camp side quests, local contracts, and path-exclusive Chapter 2 achievements.
Entering the summit at the amphitheater in Chapter 3: All Loc Muinne side quests, the dragon-related choices, and remaining spell/rune puzzles.

Recommended order

Step 1

Play the Arena tutorial mode to unlock basic combat achievements and practice parrying.
Step 2

Start Playthrough 1 on Hard or lower and side with Iorveth at the end of Chapter 1, ensuring you explore Vergen and complete all path-specific side quests, mini-games, and Chapter 3 choices.
Step 3

Farm or clean up general combat, alchemy, and crafting achievements before finishing your first playthrough.
Step 4

Start Playthrough 2 on Dark difficulty and side with Vernon Roche at the end of Chapter 1 to unlock the alternate narrative achievements and craft the exclusive Dark mode gear sets.
Step 5

Complete Insane difficulty only if you prefer it over Dark, though Dark difficulty is sufficient for the Enhanced Edition 100% completion.

Great Potion! Missable 34.6% of players Brew the ostmurk potion. Obtain the ostmurk ingredient from the waterfall cave in Flotsam and brew the Mongoose potion during Chapter 1. Where: Flotsam

Gambler Missable 28.5% of players Win an arm wrestling match, a dice poker game and a fist fight. Win at least one match of arm wrestling, one game of dice poker, and one fistfight minigame.

Fat Man Missable 25.3% of players Hidden achievement Choose Vernon Roche's path at the end of Chapter 1, assault Flotsam's residence, and kill Bernard Loredo. Where: Geralt, Chapter 1 (Roche Path)

Librarian Missable Collectible 22.1% of players Find all additional information about the insane asylum's history. Collect all four medical records and notes inside the ruined insane asylum during the "In the Claws of Madness" quest. Where: The Forest

Eagle Eye Missable 20.4% of players Hit Count Etcheverry using the ballista. Calibrate the courtyard ballista correctly during the Prologue to hit Count Etcheverry on your third aim adjustment. Where: La Valette Castle

Sensitive Guy Missable 19.9% of players Hidden achievement Break Sile de Tansarville out of the malfunctioning megascope in Chapter 3 by removing the flawed crystal instead of letting her die. Where: Geralt, Chapter 3

Man of the Shadows Missable 19.4% of players Successfully sneak through Loredo's garden and find the component of the kayran trap. Sneak past all the guards in the backyard of Loredo's compound undetected and loot the iron frame component from the chest. Where: Flotsam

Friend of Trolls Missable 18.7% of players Spare all trolls in the game. Spare the troll in Chapter 1, and spare both the male and female trolls during Chapter 2.

Old Friends Missable 18.2% of players Hidden achievement Finish Chapter 3 by siding with Vernon Roche and rescuing Triss Merigold yourself rather than saving Anais. Where: Geralt, Chapter 3

Dragonheart Missable 16.9% of players Hidden achievement Choose to spare Saskia's life after defeating her dragon form in Chapter 3 rather than executing her. Where: Geralt, Chapter 3

Backbone Missable 15.6% of players Craft a suit of armor from elements of the kayran's carapace. Loot the Kayran skin after defeating the beast in Chapter 1, buy the Carapace armor diagram from the craftsman in Lobinden, and pay him to craft it. Where: Flotsam

Gladiator Missable 15.2% of players Defeat all opponents in the Kaedweni arena. Geralt, Chapter 2 (Roche Path) | Complete the quest "The Butcher of Cidaris" and continue fighting in the Kaedweni camp arena until you defeat every challenger, concluding with Ves. Where: Kaedweni Camp

Heartbreaker Missable 14.1% of players Seduce Ves. Geralt, Chapter 2 (Roche Path) | Defeat Ves in the arena during the quest "The Butcher of Cidaris" and then talk to her at Roche's camp to successfully seduce her. Where: Kaedweni Camp

Pest Control Missable 14.1% of players Finish all quests involving the destruction of monster nests. Geralt, Chapter 3 | Complete every monster contract involving the destruction of nests across all three chapters (nekker and endrega in Chapter 1; rotfiend and harpy/gargoyle in Chapters 2 and 3). Where: Loc Muinne

Being Witcher George Missable 11.1% of players Hidden achievement Geralt, Chapter 3 | Defeat the dragon at the top of the tower during the climax of Chapter 3 and choose to deliver the mercy killing instead of walking away. Where: Loc Muinne

Avenger Missable 10.5% of players Hidden achievement Geralt, Chapter 3 | Confront Letho in the epilogue at the Loc Muinne courtyard and defeat him in the final duel rather than letting him walk away. Where: Loc Muinne

Black Ops Missable 10.4% of players Sneak through the lower camp without raising the alarm. Geralt, Chapter 2 (Iorveth Path) | Sneak across the lower Kaedweni camp unnoticed without alerting any guards while retrieving Triss's figurine during Chapter 2. Where: Kaedweni Camp

Tourist Missable 10% of players Tour the camp with Zyvik. Geralt, Chapter 2 (Roche Path) | Follow Zyvik on his complete guided tour through the Kaedweni military camp without leaving or interrupting the dialogue. Where: Kaedweni Camp

Spellbreaker Missable 9.8% of players Hidden achievement Geralt, Chapter 3 | Choose to help Iorveth rescue Philippa Eilhart in Chapter 3 to lift the curse placed upon Saskia. Where: Loc Muinne

Witch Hunter Missable 9.8% of players Hidden achievement Geralt, Chapter 3 | Choose to rescue Anais La Valette with Vernon Roche in Chapter 3 rather than going to save Triss Merigold. Where: Loc Muinne

Necromancer Missable 8.1% of players Hidden achievement Geralt, Chapter 2 (Roche Path) | Help Dethmold perform the necromantic ritual on the assassin Auckes's corpse and successfully navigate the memory sequence. Where: Kaedweni Camp

Summer Solstice Missable 7.9% of players Hidden achievement Geralt, Chapter 1 | Find the Shrine of the Sun in the woods outside Flotsam and interact with it to receive the Sun buff. Where: The Forest

Artful Dodger Missable 7.8% of players Cut off a tentacle using the kayran trap. Craft the Kayran trap using the iron frame found in Loredo's compound, place it in the marked glowing zone during the Kayran fight, and lure a tentacle into it. Where: Flotsam

Winter Solstice Missable 7.3% of players Hidden achievement Geralt, Chapter 2 (Iorveth Path) | Discover and interact with the Shrine of Winter located in the valley outside Vergen to obtain the Winter buff. Where: Vergen

Kingmaker Missable 6.9% of players Hidden achievement Geralt, Chapter 3 | Complete the Chapter 3 questline on Roche's path by ensuring Anais La Valette is safely delivered to either King Radovid or John Natalis. Where: Loc Muinne

Reasons of State Missable 6.3% of players Hidden achievement Geralt, Chapter 3 | Complete Roche's path questline in Chapter 3 and decide the political fate of Temeria by choosing whether to kill or spare Bernard Loredo/Henselt earlier and resolving Anais's custody. Where: Loc Muinne

Trollslayer Missable 2.9% of players Kill all the trolls in the game. Kill the troll in Flotsam (Chapter 1), both trolls in the ravine in Chapter 2, and the female troll in Loc Muinne (Chapter 3). Where: Loc Muinne

Perfectionist Cumulative 66.4% of players Kill 10 foes in a row without losing any Vitality. Defeat 10 consecutive enemies without taking any damage to your Vitality.

Taster 54.8% of players Drink one potion. Enter meditation and drink any single potion from your inventory.

The Fugitive 51.7% of players Finish the Prologue. Complete the Prologue and escape from the dungeons beneath La Valette Castle. Where: Geralt, Prologue

Intimidator 42.1% of players Intimidate someone. Successfully use the Intimidation dialogue option during any conversation.

Craftsman 37.5% of players Hire a craftsman to create an item. Pay any blacksmith or craftsman to craft an item using a diagram.

Journeyman 34.4% of players Achieve character level 10. Earn enough experience points across your playthrough to reach character level 10.

Oh My God! You Killed the Kayran! You Bastards! 34.1% of players Hidden achievement Defeat the giant Kayran monster at the end of the main quest in Chapter 1. Where: Geralt, Chapter 1

Apprentice 32.6% of players Use alchemy to brew five potions or oils. Enter meditation and brew any combination of five potions or oils via alchemy.

To Aedirn! 31% of players Complete Chapter 1. Complete the final quests of Chapter 1 and depart Flotsam for the Upper Aedirn region. Where: Geralt, Chapter 1

The Butcher of Blaviken Cumulative 25.9% of players Kill 500 foes. Slay a cumulative total of 500 enemies across the entire game.

Alea Iacta Est 25.1% of players Complete Chapter 2. Finish all main storyline quests in Chapter 2 on either Roche's or Iorveth's path. Where: Vergen

To Be Continued... Difficulty 23.4% of players Finish the game at any difficulty level. Complete the Epilogue and finish the entire game on any chosen difficulty setting. Where: Geralt, Epilogue

Once Ain't Enough 23.3% of players Reach the Epilogue of the game. Complete Chapter 3 in Loc Muinne to advance into the Epilogue. Where: Geralt, Chapter 3

Swordmaster 21.5% of players Acquire the Combat Acumen ability. Invest points into the Swordsmanship tree until you unlock the tier-6 "Combat Acumen" ability.

Torn Asunder! 21.5% of players Kill more than one opponent using a single exploding bomb. Throw an explosive bomb such as Grapeshot into a cluster of enemies so that the blast kills at least two simultaneously.

Ricochet 16.6% of players Kill a foe by deflecting his own arrow at him. Unlock level 2 of the Arrow Redirection skill in the Training tree, then parry an incoming enemy arrow so it reflects back and kills the archer.

Threesome 16.1% of players Kill three foes at once by performing a group finisher. Unlock the Whirl skill in the Swordsmanship tree, charge your adrenaline to full in combat, and activate your group finisher on three nearby enemies.

Mutant! 12.9% of players Enhance abilities using mutagens at least five times. Acquire at least five mutagens and slot them into abilities that feature mutagen slots across your character development trees.

Guru 11.4% of players Achieve character level 35. Earn enough experience points by completing quests, contracts, and defeating foes to reach the maximum character level of 35.

Master of Magic 10.9% of players Acquire the Sense of Magic ability. Invest points into the Magic skill tree until you unlock the final ability, Sense of Magic.

Focus 9% of players Perform three successful ripostes in a row. Unlock the Riposte skill in the Swordsmanship tree and successfully counter-attack three consecutive enemy strikes without breaking the chain.

Master Alchemist 6.6% of players Acquire the Mutant ability. Invest points into the Alchemy skill tree until you unlock the tier-two ability named Mutant.

Miser 6.4% of players Collect 10000 orens. Accumulate a total balance of 10,000 orens in your inventory at one time through quest rewards, gambling, and selling loot.

Poker! 5.8% of players Roll five-of-a-kind at dice poker. Play a match of dice poker against any opponent and roll five identical dice faces in a single throw.

Madman Difficulty 2% of players Finish the game while playing at the Insane difficulty level. Complete the entire main story on Insane difficulty without dying, as death permanently deletes your save file. Where: Geralt, Epilogue
