Agreeing to fly to Orbit Elevator / Spire 4 with Adam: Access to Xion, Wasteland, and Great Desert side quests closes permanently, and you are locked out of visiting Eidos 9 if Lily's progress bar is not full.
Entering the Nest at the end of the game: Locks you out of returning to Spire 4 or earlier exploration camps, forcing you into the endgame choices and final battles.
Choosing whether to accept or refuse Adam's hand at the final encounter: Locks you into specific ending achievements for that playthrough once the credits roll and auto-save updates.

Step 1

Play through the campaign while maxing out Lily's affinity meter to 100% by picking up documents, memory sticks, and completing side quests across Eidos 7, Wasteland, Matrix 11, and Great Desert.
Step 2

Finish all side quests in Xion, Wasteland, and Great Desert before agreeing to travel to Orbit Elevator / Spire 4 with Adam.
Step 3

Travel to Eidos 9 via the story route (unlocked automatically at 100% Lily meter) and gather all collectibles in Eidos 9 and Spire 4.
Step 4

Before the final boss sequence, choose to either upload your save to the cloud/USB to see both ending paths, or prepare to complete the second path in New Game+.
Step 5

For the first final choice, choose to Take Adam's Hand to trigger the final boss and unlock the "Making Memories" ending achievement.
Step 6

In New Game+ (or by reloading a backup before the final choice if you died without resting at the final camp), choose to Refuse Adam's Hand to unlock the "Return to the Colony" ending achievement.

In a playthrough where you purposely keep Lily's affinity meter below 100% (by ignoring collectibles and optional side content), choose to Take Adam's Hand at the end to unlock the "Cost of Lost Memories" ending achievement.

Clean up any remaining cumulative combat, fishing, Naytiba, or upgrade achievements in New Game+.
